Requests distribution(s) from your Charitable Gift Fund. Please note you may also request grant distributions by creating an Access My Fund account. Please click here to go to the new user registration page.

Change of Designated Organizations Form (Word Doc)
Allows you to modify the nonprofit organizations designated as recipients of distributions from your Endowed Gift Fund. Please use this form if you have or are adding multiple beneficiaries.
Change of Designated Organization Form (One Beneficiary, Word Doc)
Allows you to modify the nonprofit organization designated as the sole recipient of distributions from your Endowed Gift Fund. Please use this form if you have and are changing only one beneficiary.
Change of Succession Plan Form (Word Doc)
Advises the Foundation of changes in your succession plan for your Charitable Gift Fund. If you wish to add or change successor advisors, please use the Advisor Information Form below.
Gift Fund Name Change Form (Word Doc)
Advises the Foundation on changes to your Gift Fund Name.
Advisor Information Form (For Individual & Family Funds, Word Doc)
Advises the Foundation on changes to individuals authorized to act on behalf of the Gift Fund, including the addition or changing of advisors, statement recipients, and successor advisors.
Advisor Information Form (For Nonprofit & Corporate Funds, Word Doc)
Advises the Foundation on changes to individuals authorized to act on behalf of a corporate or nonprofit entity, including the addition or changing of advisors, statement recipients, and successor advisors.

Provides information about real estate property to be gifted to the Foundation.
